Nolan's Story: Overcoming Retinoblastoma and Raising Awareness for Pediatric Cancer

Nolan Smith is an inspiring nine year old who was diagnosed with retinoblastoma at just three years old.
Nolan’s mom, Courtney, a pediatrician at CHRISTUS Children’s Hospital in San Antonio, noticed something unusual in his left eye—a subtle difference in the reflection and color when the light hit it just right.
After seeking further evaluation, a pediatric ophthalmologist referred the family to pediatric retina specialist Dr. C. Armitage Harper III . A sedated eye exam, brain MRI, and genetic testing confirmed the diagnosis: sporadic retinoblastoma, a non-hereditary form of cancer that starts in the retina. Dr. Harper’s calm and thorough approach provided the family with clarity and a treatment plan during a profoundly overwhelming time.
“You form a special bond with someone who tells you your three year old has cancer and keeps you calm,” Courtney said. “Dr. Harper let me just be a mom, not a second doctor, and guided us every step of the way.”
Shortly after his diagnosis, Nolan underwent surgery to remove his left eye (enucleation) and as much of the optic nerve as possible while preserving the muscles to accommodate a prosthetic eye. Pathology results revealed that the cancer had invaded enough of the eye to necessitate chemotherapy, which Nolan completed in February 2020, just before his fourth birthday.
Dr. Harper closely monitored Nolan with sedated eye exams and MRIs every four months for the first few years after his diagnosis. Now that he is cancer free, Nolan follows up with Dr. Harper every six months.
“It’s an honor and a privilege being a physician who is intimately involved with small children who have complicated retina issues,” says Dr. Harper. “The medical conditions are sometimes very complex and require multiple specialists in order to properly diagnose and treat these conditions.”
Dr. Harper says the process of “leaning in” is just as important to provide a sense of comfort to the child and parents.
Nolan’s Impact on Pediatric Cancer Awareness
Nolan’s story didn’t end with beating cancer. The family’s involvement with CHRISTUS Children's Foundation led to Nolan being named the 2023 Children's Miracle Network Champion for San Antonio. This role allowed him to share his journey and raise awareness for pediatric cancer treatment and the critical role of children’s hospitals.
Nolan’s energy, positivity, and determination have inspired many. Despite the challenges he faced, Nolan loves school, playing video games, swimming, and building with LEGOs. He dreams of becoming an astronaut or a scientist when he grows up.
In 2024, Nolan’s advocacy reached a national stage when he was selected as one of 10 Children's Miracle Network National Champions. One of Nolan’s favorite opportunities as a CMN ambassador was partnering with 7-Eleven to design the National Slurpee Day cup found in 7-Eleven’s across the country to help raise awareness around the need for charitable donations to support pediatric healthcare.
His efforts have brought smiles and hope to countless families navigating similar challenges.
A Parent’s Perspective on Advocacy
Throughout Nolan’s journey, his family has taken time to reflect on the challenges they faced and the lessons they’ve learned. Courtney emphasizes the importance of regular well visits and trusting your instincts as a parent. “If you notice something abnormal, don’t be afraid to ask questions,” she advises. Looking back, she knows that early detection and prompt action were crucial in Nolan’s case, and she encourages other parents to advocate for their children’s health.
Dr. Harper recommends every child have an examination with a pediatric ophthalmologist by 9 months of age, if not before.
